THIS NOTICE DESCRIBES HOW HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

We create a record of all the medical, pharmacy or mental health services that you receive at Pratt Community Health Clinic. This record contains information about your symptoms, examinations, test results, x-rays, diagnoses, treatment, our plan for future care and the services we have provided. At Pratt Community Health Care, we respect our patients and their personal information. We are committed to protecting the privacy of patient records. We are also required by state and federal laws to maintain the privacy of protected health information. One of the requirements of the federal Privacy Rules is to provide patients with a Notice of Privacy Practices. This notice tells how we may use your patient information and how it may be disclosed to others. It also explains your rights and some of our legal obligations regarding your health records.

Notice of Non-Discrimination

Pratt Community Health Clinic welcomes all patients. We do not discriminate because of race, creed, sex/gender identity, sexual orientation, national origin, religion, disability or source of payment. We follow all applicable laws such as Section 1557 of the Patient Protection and Affordable Care Act and the American Disabilities Act.

 

We provide free interpretation services. We offer language services who do not speak English. Materials are written in other languages. We provide American Sign Language (ASL) interpretation for people with hearing disability. If you need these services, please call Pratt Community Health Clinic at 620-388-0462.
