Pratt Health Clinic is committed to you. Treating your healthcare needs and getting you on a path to better health regardless of insurance. We want to become your healthcare home. What does this mean? It means that we will work with you and will always respect your healthcare choices. We will offer you the best education and support services you need to make well informed decisions about your health and care.

We would be honored to coordinate your care with additional services when it becomes necessary including current primary care doctors, specialist referrals, mental health support services, hospital stays, home health care, community services, and other support services.

When you become a Direct Care Member, we promise you will have access to our services with shorter wait times, access to after hours care, 24/7 confidential communication via text, video, or telephone access.

Our providers work as a team to ensure we meet all of your health care needs. With over 15 years of experience, Pratt Community Health Clinic serves patients from Finney, Ford, Edwards, Kiowa, Kingman, Barton, Stafford, & Pratt counties.  Our commitment will always be to look for ways to increase our capacity so we can help meet the needs of the ever increasing underserved population in our community.

 

For medical we accept most commercial insurances, Medicaid and Medicare. We also see patients who donot have insurance.

Your Patient Rights

Our primary focus is to improve the lives of our patients through our services, our partnerships with our patients and community members. We want our patients to become more knowledgeable and responsible for their own health, and our commitment to wellness both emotional and physical. We are advocates for our patients.

As a patient at PCHC, you have specific rights. Some of these rights are guaranteed by federal law, such as the right to get a copy of your medical records, and the right to keep them private. In today’s world, knowing your responsibilities is key to taking control of your health with your healthcare team.
